Fella is optimized for busy bigger guys who want a no-nonsense solution to getting healthier. Delilah is built for strong women in their prime who want a sustainable solution to feel their best.

We use affordable, effective weight loss medication to solve our customers' most pressing healthcare need, then aim to remain their core healthcare provider for the rest of their life — expanding to hormonal health, mental health, longevity, and primary care.

GLP-1 medications are a f*ing big deal. We're in a remarkable & rapid period of change which will radically reshape the patient & cash flows in the healthcare system.

Our mission is to accelerate the global take-up of GLP-1 medications among busy families to help solve obesity worldwide. From this, we'll become the world's leading consumer healthcare brand.

We've grown >1000x since Aug 2021, >100x since Aug 2022, >10x since Aug 2023. We're >$50M ARR. We've been cashflow positive for the last 24mo. We're >10,000 customers with high NPS.

Almost all of this has so far been through Fella. Delilah is still very new.

We're a young startup and we're just getting started. This is still Day 1 and all our work is ahead of us.

The opportunity as Nurse Practitioner

Fella Health is building a world-class clinical team — and this is our first hire in Canada.

As a Telemedicine Nurse Practitioner, you'll help us bring our proven U.S. care model to Canadian patients, starting with async GLP-1 prescribing, then expanding to become the biggest telehealth clinic in Canada.

This isn't just a clinical role — it's a chance to shape Fella's Canadian care model from the ground up. We've scaled rapidly in the U.S., and Canada is our next major market. You'll be our first NP on the ground, and you'll help define what great care looks like here.

You'll work closely with our future Canadian Medical Director, Clinical Ops Lead, and Founder. You'll also help us shape protocols, workflows, and our overall clinical experience in Canada.

Over time (this is optional), you can transition to a senior member of the clinical team, shaping Fella's future.

This is a part-time, remote, contractor role ~20hrs/wk.

Key Responsibilities

Right away:

  • Be the responsible NP for reviewing async GLP-1 consultations and issuing prescriptions under Canadian standards.
  • Conduct streamlined sync consultations for clinical protocols where needed.
  • Flag edge cases and refine eligibility workflows (e.g. lab review, contraindications, documentation gaps).
  • Collaborate with our Ops team to debug and improve EMR workflows, careteam handoffs, and patient communication systems.
  • Help ensure we are practicing in line with provincial NP scope, prescribing standards, and regulatory expectations.
  • Build a feedback loop between patient care and our clinical documentation, SOPs, and protocol logic.

As our Canada NP:

  • Help us scale a compliant, high-quality model across provinces as we grow.
  • Contribute to provider onboarding SOPs, share what "great" looks like, and prepare the path for future NP hires.
  • Work with leadership to determine when to expand sync capacity, and what to automate vs. escalate.
  • Be our early clinical voice in Canada — partnering with Product, Ops, and Legal to shape a defensible care model.

Need to Have

  • Hours: you're able to work at least 20 hours/week
  • License: Active, unrestricted NP license in at least 3 Canadian provinces (BC, ON, AB)
  • Async: you are comfortable with an async approach in telemedicine.
  • Patient-centric: you are onboard with safely-practiced, compliant, low-touch patient-centric (not provider-centric) care.
  • Bio-first: you align with a biology-first approach, understanding your role is to safely prescribe high-efficacy GLP-1s to your patients.
  • Controlled: you're comfortable writing prescriptions for Schedule III controlled substances like TRT under a streamlined synchronous protocol.

Nice to Have

  • Obesity medicine experience: You've treated patients with GLP-1s or similar medications for obesity or weight-related conditions.
  • Men's health or longevity experience: You're comfortable with topics like testosterone, metabolic health, or longevity medicine.
  • Clinical leadership: You've helped build protocols, trained other NPs, or want to help shape a growing telemedicine care team

Our cultural standards

These are the core cultural standards to which we hold ourselves & our team-members at Fella:

  • : We will have served 100 million patients by the end of 2035 and we transform the life of most patients who join. We have a lot of work to do. We are obsessed with our patients and are dedicated to the mission.
  • : We are at the frontier, so we often live in ambiguity with no trodden path. When we can't look to others for guidance, we must maintain impeccable ethics and unwavering integrity.
  • : Bad sh*t is coming. By joining us, you're choosing to sail straight towards the storms with unhesitating conviction. However much we've already done, however far we've already come — it's still Day 1 and all our work is ahead of us.
  • : This will be your most challenging, most rewarding role of your life. You'll be shocked how capable you are & how much you can achieve.
  • : We are only interested in "insanely great", a focus on the quality of our execution that in everyday life would be considered pathological. We have a dedication to excellence and reject incompetence.
  • : That which can be destroyed by the truth should be. You get full transparency from the company and the company expects full transparency from you. We never say anything about someone that we wouldn't say to them directly. We give feedback with love and do not need to protect people from fleeting physical sensations.
  • : We execute at an intensity that most people think is impossible. Speed is critical and we need things done yesterday. We all work very hard and in such a competitive world there really is no other way to win.
  • : We are frugal. We hate being wasteful and we are anti-luxury. A culture of cheapness keeps us young. We spend our cash wisely & carefully — in a way that would make our grandmas proud.
  • : The world is malleable and we shape it. We truly believe this and act accordingly. We are relentlessly resourceful and are at the mercy of no-one but ourselves.
  • : We're boring people doing exciting work. We don't chase short-term status — we ignore short-term dopamine hits and focus on what matters. Outsiders will underestimate us and we revel in that.
  • : We live in a world of power laws and we cannot overestimate the unimportance of practically everything. Know your One Thing, and nail it.

"You just build a fing amazing experience. Make each step amazing. Make every decision in the long term interest of the customer. Give the customer massively more value than you take."*
